Congratulations! Your company has been nominated as a great place to work. In order to qualify for a "Best Places to Work in Southern Nevada®" award, we require that you provide more information about your organization and its HR practices.
To enter the competition, you must complete and submit all portions of this form no later than August 21st, 2015 by 5:00 p.m. ONLY ELECTRONIC SUBMISSIONS WILL BE CONSIDERED. Be sure your form is submitted by the deadline.

Are you the individual responsible for filling out the awards entry form on your company's Human Resource practices? If not, please go back and submit the Nomination form first. Then notify your HR Director to fill out this Awards Entry Form.
Please be sure to answer all questions as completely as possible and provide any additional information you feel might help you earn a spot as a finalist in the program.

You can print out this page before filling it out to gather all of the answers before coming back to submit the application. Feel free to write any longer passages in Word, then copy and paste your text into this form before submitting. This will allow you to use Word's "Word count" feature to determine that the lengths of your responses are appropriate per the instructions. Longer responses than requested may be truncated for judging purposes.
